Pharmaceutical cannabis capsule dosing begins lower than most people expect. A reasonable starting point is 1 to 2.5 mg of THC, or 5 to 10 mg of CBD, taken once. Then you wait. Capsules are swallowed and digested, so they behave like edibles: onset runs 30 to 120 minutes, peak effect lands around 2 to 4 hours, and the full effect can stretch 6 to 8 hours or longer. Deciding to redose at the 45-minute mark is the single most common mistake I see.

what is the optimal dose for cannabis capsules

What counts as a pharmaceutical cannabis capsule

Two different product families get called this, and they are not interchangeable.

The first is FDA-approved. Dronabinol is synthetic THC suspended in sesame oil and sold in 2.5 mg, 5 mg, and 10 mg capsules. Nabilone is a synthetic cannabinoid analog sold in 1 mg capsules. Both are prescription-only controlled substances with narrow labeled uses: dronabinol for chemotherapy-induced nausea that has not responded to other antiemetics and for appetite loss tied to AIDS-related weight loss; nabilone for chemotherapy-induced nausea and vomiting. Their doses come from clinical trials, not from dispensary menus.

The second family is state-legal cannabis capsules made to pharmacy-style standards: standardized potency, third-party testing, batch numbers, and a real label. They are not FDA approved, and potency claims rest on the testing lab rather than on federal review.

Read the label before the dose

What I check on any capsule bottle:

  • Milligrams of THC and CBD per capsule, not per bottle
  • Ratio, such as 1:1 or 20:1 CBD to THC
  • Carrier oil, since MCT, olive, and sesame oil absorb at different rates
  • Batch number and expiration date
  • Whether it is full-spectrum, broad-spectrum, or isolate

A 10 mg THC capsule and a 10 mg THC tincture are not the same experience. Oils held under the tongue partly bypass the gut. Capsules do not.

Labeled doses for the prescription products

Dronabinol for appetite stimulation is typically started at 2.5 mg twice daily, before lunch and before dinner, with a labeled ceiling of 20 mg per day. For chemotherapy nausea, the labeled dose is based on body surface area, roughly 5 mg per square meter given one to three hours before treatment and repeated every two to four hours afterward, up to four to six doses per day.

Nabilone is usually dosed at 1 to 2 mg twice daily, with the first dose taken one to three hours before chemotherapy. These numbers assume a prescriber is managing side effects, which can include drowsiness, dry mouth, dizziness, and mood changes.

A titration schedule that actually works

  1. Pick one time of day and one dose. Take it with a normal meal, not a heavy one.
  2. Write down the effect at 1 hour, 2 hours, and 4 hours. Effects, not expectations.
  3. Hold that dose for three to four days. Cannabinoid tolerance and sensitivity shift with repeated exposure.
  4. If relief is inadequate and side effects are mild, increase by the smallest available step, often 1 to 2.5 mg of THC.
  5. Never stack a second capsule during the first one's window. Wait at least six hours.

Why the same capsule hits differently on different days

Swallowed THC goes through the liver before it reaches circulation. That first pass converts a chunk of it into 11-hydroxy-THC, a metabolite that is more potent and crosses into the brain more readily than THC itself. This is why oral cannabis can feel heavier and more sedating than inhaled cannabis at a comparable milligram count.

Bioavailability for oral THC is low and variable, often cited in the range of roughly 6 to 20 percent. A fatty meal can raise absorption. A empty stomach can blunt it. Liver health, other medications, and individual enzyme activity all move the number around.

Interactions and people who need more caution

Cannabinoids are metabolized by CYP3A4 and CYP2C9 enzymes, so the list matters:

  • Warfarin and other anticoagulants
  • Clobazam, valproate, and other anticonvulsants
  • Ketoconazole, rifampin, and similar enzyme inducers or inhibitors
  • Opioids, benzodiazepines, sleep aids, and alcohol

Older adults, anyone with liver disease, and people who are pregnant or breastfeeding should talk to a clinician before starting. Driving is off the table until you know how a dose affects you, and that can take a week of consistent use. Store capsules locked and out of reach of children and pets.

What to log

Date, time, milligrams of THC and CBD, whether you ate, how you felt at one, two, four, and eight hours, and how you slept. After two weeks you will have a personal dose curve instead of a guess. Bring that log to your pharmacist or prescriber. It makes the conversation faster and the adjustments smarter.

This is general information, not medical advice. If you take prescription medication or have a health condition, dose with a clinician in the loop.
