Answer

THC, the main psychoactive compound in cannabis, binds to CB1 receptors in the brain and can raise hunger. The effect lasts a few hours per dose. It does not fix malnutrition on its own. Two FDA-approved THC medicines, dronabinol and nabilone, are prescribed for nausea and for appetite loss in AIDS patients. Data on weight gain in other groups is mixed.

How THC Triggers Hunger

CB1 receptors sit in the hypothalamus and the nucleus accumbens, two areas that govern food intake and reward. THC attaches to these receptors and raises the response to the smell and taste of food. Rodent work published in Nature Neuroscience found THC acts on olfactory circuits and sharpens odor detection. The hormone ghrelin, which signals hunger, rises in some users after use.

What the Clinical Data Shows

  • Dronabinol (Marinol), a synthetic THC capsule, received FDA approval in 1985 for anorexia linked to weight loss in AIDS patients.
  • Trials in AIDS patients recorded higher appetite scores. Weight gain was small, about 1 kg to 2 kg.
  • Trials for cancer anorexia-cachexia gave mixed results. Several found no weight gain against placebo.
  • Nabilone (Cesamet) is approved for chemotherapy nausea, not for appetite.
  • No trial has shown that a strain name, terpene profile, or Indica/Sativa label changes appetite more than THC content does.

Dose and Route

Oral THC in clinical use starts at 2.5 mg twice daily and goes up to 10 mg twice daily. A capsule or edible takes 30 to 90 minutes to act and lasts 4 to 8 hours. Inhaled cannabis acts in 2 to 10 minutes and lasts 1 to 3 hours. Edible doses above 10 mg raise the chance of nausea, dizziness and anxiety, and those effects can stop a person from eating.

Limits and Side Effects

Common THC side effects include dry mouth, drowsiness, dizziness, memory trouble and a fast heart rate. NIDA estimates about 3 in 10 people who use cannabis develop cannabis use disorder. Daily high-dose use can cause cannabinoid hyperemesis syndrome, which brings repeated vomiting and a drop in food intake. Cannabis also raises heart rate, a factor for people with heart conditions. CBD has low affinity for CB1 receptors, and rodent studies on CBD and food intake point in both directions. THCV, a minor compound, may lower appetite at high doses in animal work. Human data is limited.

Legal Status

Cannabis is Schedule I under federal law. Dronabinol and nabilone are controlled prescription drugs, not retail products. Some state medical programs list appetite loss or wasting syndrome as a qualifying condition. Check state rules before purchase.
